It is important to keep your child up to date on required immunizations for school and child care. Vaccinations can reduce the severity of disease or prevent disease entirely. The page has all the forms families need to meet immunization requirements.

Access your Family's Immunization Information
Sign up for MyIR Mobile by going to myirmobile.com and follow the registration instructions. Your registration information will be used to match your records with the state immunization registry. You will be sent a verification code on your phone to finalize the process. Once registration is complete, you'll be able to view your immunization records, Certificate of Immunization Status (CIS) and access your COVID-19 vaccination certificate.

- The pharmacies ask that you call ahead and check for availability of the vaccine prior to going in.
- We have no pharmacies in town that offer state supplied vaccines, all of them will bill your insurance or you have the option to pay cash.
- Vaccinations are also available from your family physician, call ahead to see if an appointment is needed.
